Registered Nurse (RN) – Remote Care Management Coordinator (Wound Care / Home Health / DME)
Job Summary We are seeking a dedicated and skilled Registered Nurse (RN) to join our wound care and coordination team. The ideal candidate will have a strong background in clinical care, documentation, and coordination across multiple healthcare settings. This role involves supporting mobile wound care providers by reviewing charts, managing referrals, processing labs, and ensuring accurate clinical documentation for compliance and quality patient outcomes. Responsibilities • Conduct chart reviews and documentation audits to ensure completeness and compliance with wound care and Medicare standards. • Coordinate home health referrals, labs, imaging, and DME orders for wound and lymphedema patients. • Review lab and wound culture results, flag abnormalities, and communicate updates to providers. • Support clinicians with plan-of-care updates, follow-up scheduling, and order tracking. • Communicate with home health agencies, DME vendors, and pharmacies to facilitate timely care. • Maintain accurate EMR documentation in CharmHealth and ensure all uploads meet billing criteria. • Educate patients and caregivers (via provider coordination) on wound management and supply use. • Uphold strict HIPAA compliance and confidentiality in all workflows. • Participate in quality assurance reviews and workflow improvement projects. Experience & Skills Candidates should demonstrate: • Active RN license (Maryland, D.C., or compact state). • 2+ years of clinical experience in wound care, home health, long-term care, or care coordination. • Familiarity with Medicare documentation, wound staging, and DME processes. • Strong communication and multitasking skills with attention to detail. • Proficiency with EMR systems (CharmHealth preferred; Epic, Athenahealth, or eClinicalWorks acceptable). • Understanding of ICD-10 coding, CPT billing codes, and wound supply documentation. • Confidence in reviewing clinical data such as ABIs, wound measurements, and progress photos.
