About the Role:
The Transfer & After-Hours Coordinator – Level 2 is responsible for coordinating safe, timely, and efficient inpatient transfers and after-hours operations to support in-network utilization and continuity of care.
This role serves as a key operational liaison between hospital emergency departments, hospitalists, skilled nursing facilities, bed control departments, transportation services, and internal ICM/UM teams. The coordinator ensures that transfer workflows are executed accurately, documented thoroughly, and aligned with organizational and regulatory standards. The position supports both business hours and after-hours operations (evenings, weekends, and holidays as assigned), ensuring seamless handoff, escalation, and follow-through of transfer activities.

What You'll Do
- Serve as primary point of contact for inbound transfer-related calls from emergency departments, facilities, providers, and internal teams
- Coordinate inpatient transfers of dual/global risk members from out-of-network facilities to in-network hospitals when clinically appropriate
- Facilitate real-time communication between IPA hospitalists and external providers via secure communication platforms
- Coordinate with bed control departments to secure accepting beds following hospitalist approval
- Arrange appropriate medical transportation (non-emergency ambulance) in accordance with facility protocols
- Monitor transfers to completion and confirm arrival, bed assignment, and ETA communication with accepting hospitalist
- Escalate complex cases to on call/back-up Case Manager as needed
- Ensure OB cases in active labor are managed per established protocol (no transfer required)
- Ensure no denial of emergency treatment; all transfers must be clinically approved by on-call hospitalist
- Endorse pending transfers and open issues to incoming coordinator to ensure continuity
- Work on assigned schedule by his/her supervisor or manager including alternating weekend and holidays
- This position shift Monday – Friday 7pm – 3:30 am w/ alternating weekend and holiday schedule
- Adapt to changing priorities and assume additional responsibilities across business lines or IPAs as directed by management
Documentation & Reporting (Primary)
- Maintain accurate and detailed logs of all calls and transfer activity
- Submit daily transfer/on call logs the next business day
- Maintain tracking spreadsheets and documentation in accordance with department standards
- Support reporting related to transfer volume, success rate, and timeliness
- Submit monthly billing logs for approval (if applicable)
- Maintain strict confidentiality at all times
Collaboration & Compliance
- Comply with all CM/UM policies and procedures
- Follow all facility-specific and transportation protocols
- Support compliance with regulatory and delegated model requirements
Qualifications
- High School diploma or equivalent required
- Minimum 2 years of experience in one of the following:- Hospital Emergency Department (Unit Secretary or similar role)
- Managed care environment (IPA/MSO)
- Healthcare provider coordination (home health, SNF, hospital operations)
- Knowledge of medical terminology and coding (CPT, HCPCS, ICD)
- Proficient in Microsoft Office essentials i.e. Outlook, Word, Excel, Share Point, PowerPoint and care management systems (e.g., EZCAP)
- Strong verbal and written communication skills
- Ability to multitask in fast-paced environment
- Strong organizational and problem-solving skills

Environmental Job Requirements and Working Conditions
- This position is remotely based in the U.S. Position may require evenings, weekends, and holiday coverage based on assigned On-Call schedule.
- Position will require in-person training for onboarding efforts. The office is located in 1600 Corporate Center Dr., Monterey Park, CA 91754.
- The total compensation target pay range for this role is: $22.04 - $26.00 per hour. Actual compensation will be determined based on geographic location (current or future), experience, and other job-related factors.
