Please note the position requires you to meet patients in their homes throughout Polk County, FL and you will be expected to cover a territory radius of 50 minutes from your home address. - Clinical Care & Patient Management: - Conduct in-home visits (minimum 25/week), including assessments, exams, and medication reviews. - Manage chronic conditions and develop individualized treatment plans. - Maintain accurate EMR documentation and comply with healthcare regulations. - Patient Engagement & Collaborative Care - Educate patients on disease management and healthy living. - Collaborate with interdisciplinary teams to ensure coordinated care.   Who You Are Required Qualifications: - Active, unrestricted current licensure as a Provider in the state in which you will be providing care for patients. License must be clear of any clinical and/or board practice restrictions. - Certification as a Family Nurse Practitioner, Adult-Gerontology Nurse Practitioner or Physician Assistant - Obtain autonomous provider status as required by state you are licensed with currently within six months of meeting eligibility requirements. Preferred two years of experience in a primary care or longitudinal care setting. - Ability to pass background check upon hire and throughout employment to include criminal felony & misdemeanor search, SSN validation/trace search (LEIE), education report (highest degree obtained), civil upper and lower search, 7-year employment report, federal criminal search, statewide criminal search, widescreen plus national criminal search, health care sanctions-state med (SAM), national sex offender registry, prohibited parties (OFAC) (terrorist watchlist), and a 10-Panel Drug Screen. - MVR with reliable transportation and clean driving record. - DEA license and BLS certification Living our Values: - Serving everyone with compassion and leading with empathy. - Stepping up and creating value by taking charge and acting when there is an opportunity. - Adapting in a changing world by recognizing our responsibility to be agile and respond quickly. - Nurturing growth and belonging by respecting and celebrating everyone for who they are. Competencies - Must be a self-motivated professional who enjoys working independently and making a meaningful impact on member health. - Have a strong understanding of risk adjustment models for the Center for Medicare and Medicaid Services (CMS) and the Affordable Care Act (ACA). - Ability to document assessment findings accurately and thoroughly - Maintain patient, team member and employer confidentiality while remaining in compliance with all HIPAA regulations. - Demonstrated clinical skills necessary to perform in-home assessments Working conditions: - High-speed internet service (cable or fiber optic) with minimum download Speed of 20 Mbps, Upload Speed of 5 Mbps, and Maximum Latency of 100 milliseconds (must be installed before starting) required. - Frequent use of computer and phone systems - Must be able to constantly remain in a stationary, sitting position, communicate and exchange information with others, inspect information, perform repetitive motions with arms and fingers, interpret data, problem solve, make decisions, organize and plan, and maintain a positive and professional attitude in all situations - Ability to lift and carry up to 20 lb - Work is performed in patient homes or facilities and may involve exposure to varied conditions. - Regular travel to patient homes, provider facilities, or community locations.
