Accolade's Virtual Primary Care Department is Currently Recruiting for a Full Time Virtual Primary Care Registered Nurse. We're hiring for the start date of October 13th. This position is Remote and the applicants will be required to work from 1:00pm to 10:00pm EST. This position has an ON CALL rotating schedule every 4 to 5 weeks for a 7 day period. Role overview As a Virtual Primary Care Registered Nurse, you will be a vital member of the AccoladeCare team, playing a significant role in managing acute and chronic conditions. Creating lasting relationships is the foundation from which our AccoladeCare team is derived. Accolade Personalized Healthcare has a network of "virtual first" primary care providers (vPCP). Our AccoladeCare team will support our patients in "virtual first" primary care or local primary care delivery when appropriate or requested. Our goal is to be proactive by getting to know our patients personally, which will allow us to support them at a holistic human level. A day in the life… OPERATIONAL: • Leverage technology to proactively outreach to patients with complex, chronic medical conditions at a set frequency to monitor for self-management, reinforce disease management and identify the need for early intervention • Serves as the primary contact for patients in need of clinical support. • Identify health conditions needing urgent intervention, determine next best steps, and escalate patient needs when appropriate • Correspond with patients in whatever method is most suitable for them (phone, email, in-app) • Continuously educate patients to understand their treatment plans and when to contact their provider • Evaluate acuity to prioritize patient needs for acute and high-risk patients alongside Health Coach within the AccoladeCare team • Accompany and collaborate with critical external stakeholders and care team members during an acute or high-risk patient’s transition of care • Guide patients to resources to educate them on their specific health conditions • Assure all acute or high-risk patients have an accurate and current care plan and ensure the AccoladeCare team is executing them • Ability to identify key trigger points to smoothly transition patients back to vPCP after referral/s or acute healthcare needs • Be available when patients need us • Follow-up and do the leg work when appropriate • Help patients to address barriers to care • Learn about the lives of patients allowing us to customize the support we provide to them and their families • Provide options to care and support patients when making treatment decisions • Educate patients to be effective/efficient consumers of healthcare by preparing them for visits, tests, and procedures • Educate and empower patients, so they confidently navigate the healthcare system • Analyze the impact of patient engagements to provide meaningful data back to patients • Any other duties needed to help drive to our Vision, fulfill our Mission, and abide by our Organization’s Values What we are looking for… • Must have a compact state Registered Nurse (RN) license with a minimum of 4 years (experience in cardiac/pulmonary, oncology, med surg (GI), or BH is a plus) • Willingness to be cross-licensed in states not covered by a compact license • Care Coordination & Transition Management Certification a plus • Possesses excellent problem-solving skills and an ability to overcome systematic hurdles to fulfill patient needs • Help patients identify complications and signs/symptoms that can lead to hospital admissions and readmissions • Exceptional collaboration skills • Outstanding understanding of chronic disease management, complex medical conditions, and conducting phone assessments to assess chronic conditions • Experience and comfort with multitasking • Contentment with uncertainty • Effective communication and teaching skills • Ability to listen, talk and type all at once • Empathetic, critical thinking • Commitment to quality and continuous improvement • Demonstrated ability to use Microsoft Office (Word, SharePoint, PowerPoint) • Strong self-management • Excellent written and verbal communication skills with proven ability to communicate with individuals at various levels within the organization • Ability to perform well under pressure, adapt to change, and meet deadlines in a fast-paced, dynamic, evolving environment • Aptitude for critical thinking/problem solving • Demonstrated attention to detail • Exemplifies compassion while meeting business needs • Adaptation; champion of change • Data-driven and fact-based: focused on getting to the best answer for clients • Detail-oriented, inquisitive, problem-solving in nature • A team player capable of working effectively with individuals throughout the organization What is important to us Creating an enduring company that is hyper-focused on our culture and making a meaningful impact in the lives of our employees, members and customers.
